Register for Benefits

To register for veterans’ benefits, service personnel must have their Military ID if still active or the original DD214 (proof of military service) issued at time of discharge (or a copy of the DD214 that has been certified by the Cook County Recorder of Deeds’ Office at Chicago City Hall) and any medical records or exam reports available. Only the active serviceperson or discharged/retired veteran applying for benefits can register, except for those benefits for which a surviving spouse or dependent may apply, such as scholarships, burial and memorial services and life insurance.
